Charitable Contributions and Sponsorships

Axovant will consider requests for sponsorships and donations for activities and initiatives from independent patient and professional organizations that are:

  • Based in the United States
  • Classified and headquartered in the United States as a 501(c)(3) organization and seeking support for US-based programs
  • Requesting support for a specific program or for a designated purpose
  • Providing support and/or education to patients and/or caregivers in the United States and/or conducting research or support initiatives within Axovant’s priority disease areas
  • Seeking and receiving support from a variety of funding sources
  • Compliant with applicable federal and state laws and industry standards, including the Pharmaceutical Research and Manufacturers of America (PhRMA) Code

Requests for consideration must be submitted at least 30 days before related activities are to begin to and are required to include the following:

  • Summary description with clearly defined objectives and strategy of the project to be funded
  • A description of how specifically the project will benefit patient care, knowledge, or public health objectives
  • Timing, including beginning and end, of project
  • An itemized budget or other breakdown describing the proposed use of funds
  • Proof of IRS status (ie, IRS W-9 Form) for charitable contribution requests
  • A statement of independence from Axovant influence, with the recipient taking responsibility for selecting content, speakers, faculty, attendees, and logistical elements of the educational program (for charitable contribution requests)

Omission of any of the above elements or failure to comply with the 30-day submission requirement is the responsibility of the requesting party. Axovant is under no obligation to review proposals that do not meet the above criteria nor is Axovant responsible for follow-up to requesting party to solicit any of the above elements if they are missing.
